RESOLUTION RS2017-928

A resolution approving amendment one to a contract between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and the Nashville Academy of Medicine to provide access to the Charisma Salus database system for patient tracking and prescription tracking.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract with the Nashville Academy of Medicine to provide access to the Charisma Salus database system for patient tracking and prescription tracking approved by RS2016-424; and,

WHEREAS, the parties wish to amend the contract by extending the end date of the contract to June 30, 2018, a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the contract by and between the Nashville Academy of Medicine and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide access to the Charisma Salus database system for patient tracking and prescription tracking; a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
