RESOLUTION RS2017-850

A resolution approving Amendment Three to a contract for services by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and United Neighborhood Health Services, Inc. to provide homeless individuals with medical services at two or more clinics in or near downtown Nashville.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract for services with United Neighborhood Health Services, Inc. to provide homeless individuals with medical services at two or more clinics in or near downtown Nashville approved by RS2014-1294; and,

WHEREAS, the parties wish to amend the contract for services to extend the end date of the contract from June 30, 2017 to June 30, 2018; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment three be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment three to the contract by and between United Neighborhood Health Services, Inc. and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide homeless individuals with medical services at two or more clinics in or near downtown Nashville, a copy of which amendment three is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
