RESOLUTION NO. RS2016-256

A resolution approving amendments one, two, and three to a grant from the Greater Nashville Regional Council to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, to provide Nutrition HCBS (Home and Community Based Services) to eligible seniors throughout Davidson County.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, previously entered into a grant agreement with the Greater Nashville Regional Council to provide Nutrition HCBS (Home and Community Based Services) to eligible seniors throughout Davidson County approved by Resolution RS2015-1625; and,

WHEREAS, the parties wish to amend the grant agreement. Amendment One changes the amount per unit in Section C.3, Subsection b; Amendment Two increases the award by $43,000.00 for Nutrition HCBS (Home and Community Based Services) from $37,000.00 to $80,000.00; and Amendment Three corrects a typographical error in Section C.1 to reflect the correct start date of the contract of July 1, 2015, a copy of which amendments one, two, and three are attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendments one, two, and three be accepted.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendments one, two, and three to the grant by and between the Greater Nashville Regional Council and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, to provide Nutrition HCBS (Home and Community Based Services) to eligible seniors throughout Davidson County, a copy of which amendments one, two, and three are attached hereto and incorporated herein, are hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
