RESOLUTION NO. RS2016-155

A resolution approving amendment one to a contract between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and Family and Children’s Services 211 to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into an agreement with Family and Children’s Services 211 to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system approved by RS2013-755; and,

WHEREAS, the parties wish to amend the contract by amending Section 1 (Duties and Responsibilities of Contractor), Section 2 (Duties and Responsibilities of Metro), Section 3 (Term), and Section 4 (Compensation), a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the contract between Family and Children’s Services 211 and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system, a copy of which amendment one is attached hereto, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
