RESOLUTION NO. RS2015-1611

A resolution approving amendment number one to the contract between The Metropolitan Government of Nashville and Davidson County and Concentra Health Services, Inc., for the provision of medical services for employees who have sustained an occupational injury.

WHEREAS, Ordinance No. BL2013-527 approved a contract between The Metropolitan Government of Nashville and Davidson County and Concentra Health Services, Inc., for the provision of medical services to employees injured by accident arising out of and in the course of employment; and,

WHEREAS, the parties wish to amend certain provisions of the contract; and,

WHEREAS, Ordinance No. BL2013-527 provides that amendments to the contract shall be approved by resolution of the Metropolitan Council receiving at least twenty-one affirmative votes; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that this amendment to the contract be approved.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment number one to the contract by and between The Metropolitan Government of Nashville and Davidson County and Concentra Health Services, Inc., a copy of which is attached hereto and incorporated herein, is hereby approved and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
