RESOLUTION NO. RS2014-1222

A resolution approving amendment one to a grant from the Greater Nashville Regional Council to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, to provide personal care services, homemaker services, and Nutrition HCBS (Home and Community Based Services) to eligible clients under the options program.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, previously entered into a grant agreement with the Greater Nashville Regional Council to provide personal care services, homemaker services, and Nutrition HCBS (Home and Community Based Services) to eligible clients under the options program, approved by Resolution RS2014-1195; and,

WHEREAS, the parties wish to amend the grant agreement by changing the rate for meals listed in Section C.3, Subsection b with no change in the grant total, a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the grant by and between the Greater Nashville Regional Council and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Social Services Commission, to provide personal care services, homemaker services, and Nutrition HCBS (Home and Community Based Services) to eligible clients under the options program, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
