RESOLUTION NO. RS2013-776

A resolution approving amendment two to the contract for services by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and United Neighborhood Health Services, Inc. to provide health care services at the Downtown Clinic.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract for services with United Neighborhood Health Services, Inc. to provide health care services at the Downtown Clinic; and,

WHEREAS, the parties wish to amend the contract for services. The contract is extended for one additional term beginning July 1, 2013 and ending on June 30, 2014. The contractor shall be paid no less than $355,100.00 for term three of the contract, a copy of which amendment two is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment two be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment two to the contract by and between United Neighborhood Health Services, Inc. and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide health care services at the Downtown Clinic, a copy of which amendment two is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
