RESOLUTION NO. RS2013-755

A resolution approving a contract and business associate agreement between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and Family and Children’s Services 211 to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system.

WHEREAS, it is to the mutual benefit of Family and Children’s Services 211 and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system; and,

WHEREAS, Family and Children’s Services 211 and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, have entered into a contract and business associate agreement, attached hereto and incorporated herein, to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that this contract and business associate agreement be approved.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That the contract and business associate agreement between Family and Children’s Services 211 and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide an access to care model for the uninsured in Davidson County using the ServicePoint/CallPoint Database system, attached hereto and incorporated herein, is hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
