RESOLUTION NO. RS2012-325

A resolution approving amendment one to the contract for services by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and United Neighborhood Health Services, Inc. to provide health care services at the Downtown Clinic.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract for services with United Neighborhood Health Services, Inc. to provide health care services at the Downtown Clinic; and,

WHEREAS, the parties wish to amend the contract for services by extending Section 1. Term - to June 30, 2013 and Section 2. Compensation - Term Two – contractor shall be paid no less than $362,400.00, a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the contract by and between United Neighborhood Health Services, Inc. and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide health care services at the Downtown Clinic, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
