RESOLUTION NO. RS2011-1700

A resolution approving amendment one to a grant from the State of Tennessee, Department of Health, to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide care coordination services for the children’s special services program including pediatric and developmental assessment.

WHEREAS, The Metropolitan Government of Nashville and Davidson County previously entered into a grant agreement with the State of Tennessee, Department of Health, to provide care coordination services for the children’s special services program including pediatric and developmental assessment; and,

WHEREAS, the parties wish to amend the grant agreement to decrease the amount of the grant by $50,600.00 from $787,100.00 to $736,500.00; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the grant by and between the State of Tennessee, Department of Health, and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide care coordination services for the children’s special services program including pediatric and developmental assessment, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
