RESOLUTION NO. RS2011-1689

A resolution approving amendment one to a contract between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and the Matthew Walker Comprehensive Health Center for a licensed clinical social worker to provide initial behavioral health evaluations for all new Healthy Start Program participants.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract with the Matthew Walker Comprehensive Health Center for a licensed clinical social worker to provide initial behavioral health evaluations for all new Healthy Start Program participants approved by RS2010-1216; and,

WHEREAS, the parties wish to amend that contract by changing the compensation to $9,680.00 and the term end to May 31, 2012, a copy of which amendment one is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be approved.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and the Matthew Walker Comprehensive Health Center for a licensed clinical social worker to provide initial behavioral health evaluations for all new Healthy Start Program participants, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
