RESOLUTION NO. RS2010-1264

A resolution approving amendment one to a grant from the State of Tennessee, Department of Health, to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide renal intervention services to at risk individuals who are diabetic and/or hypertensive.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a grant agreement with the State of Tennessee, Department of Health, to provide renal intervention services to at risk individuals who are diabetic and/or hypertensive RS2009-852; and,

WHEREAS, the parties wish to amend the grant agreement by using Federal American Recovery and Reinvestment Act of 2009 (ARRA) funds for part of the existing grant previously funded entirely by State dollars. The State award of $171,000.00 will be reduced by $34,200.00 decreasing the State contribution from $171,000.00 to $136,800.00. The Federal (ARRA) funding portion will be $34,200.00 and amendment one also adds (ARRA) requirements to Section E15 of the grant contract; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be accepted.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the grant by and between the State of Tennessee, Department of Health, and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide renal intervention services to at risk individuals who are diabetic and/or hypertensive, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That the amount of this grant is to be appropriated to the Metropolitan Board of Health based on the revenues estimated to be received and any match to be applied.

Section 3. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
