RESOLUTION NO. RS2009-644

A resolution approving an amendment to a grant from the U.S. Department of Health and Human Services to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to enhance access to a comprehensive continuum of high quality, community-based care for low income individuals and families with HIV disease.

WHEREAS, the U.S. Department of Health and Human Services previously entered into a grant in an amount of $3,532,078.00 to the Metropolitan Board of Health to enhance access to a comprehensive continuum of high quality, community-based care for low income individuals and families with HIV disease, which grant agreement was approved by RS2008-236; and,

WHEREAS, the parties wish to amend that grant agreement to increase the amount by $33,213.00 for a new total of $3,565,291.00, a copy of which amendment is attached hereto and incorporated herein; and,

WHEREAS, it is to benefit the citizens of The Metropolitan Government of Nashville and Davidson County that this grant be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That the grant amendment by and between the U.S. Department of Health and Human Services, and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to enhance access to a comprehensive continuum of high quality, community-based care for low income individuals and families with HIV disease, a copy of which grant amendment is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized the execute the same.

Section 2. That the amount of this grant is to be appropriated to the Metropolitan Board of Health based on the revenues estimated to be received and any match to be applied.

Section 3. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
