RESOLUTION NO. RS2009-1056

A resolution approving amendment one to a grant from the American Legacy Foundation to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to target socio-economically disadvantaged adult smokers and their families to encourage smoke-free environments.

WHEREAS, the American Legacy Foundation previously entered into a grant in the amount of $86,029.00 to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to target socio-economically disadvantaged adult smokers and their families to encourage smoke-free environments which grant agreement was approved by RS2008-578; and,

WHEREAS, the parties wish to amend that grant agreement to increase the amount by $42,984.00 with an in-kind match of $42,984.00 and extend the end date from December 4, 2009 to December 4, 2010; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that amendment one be approved.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That amendment one to the grant contract by and between the American Legacy Foundation, in an amount not to exceed $42,984.00 to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to target socio-economically disadvantaged adult smokers and their families to encourage smoke-free environments, a copy of which amendment one is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same..

Section 2. That the amount of this grant is to be appropriated to the Department of Health based on the revenues estimated to be received and any match to be applied.

Section 3. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
