RESOLUTION NO. RS2008-504

A resolution approving an amendment to a grant from the Saint Thomas Health Services Fund d/b/a Safety Net Consortium of Middle Tennessee to The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to reduce socioeconomic and racial/ethnic disparities in Type 2 diabetes in adults.

WHEREAS, The Metropolitan Government of Nashville and Davidson County previously entered into a grant agreement with the Saint Thomas Health Services Fund d/b/a Safety Net Consortium of Middle Tennessee; and,

WHEREAS, the parties wish to amend the grant agreement to increase the amount of the grant by $6,800.00 from $65,700.00 to $72,500.00 and to renew the grant for an additional one year term ending October 31, 2009, a copy of which amendment is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that this amendment be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1: That the amendment to the grant by and between the Saint Thomas Health Services Fund d/b/a Safety Net Consortium of Middle Tennessee and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to reduce socioeconomic and racial/ethnic disparities in Type 2 diabetes in adults, a copy of which amendment is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2: That the amount of this grant is to be appropriated to the Metropolitan Department of Health based on the revenues estimated to be received and any match to be applied.

Section 3: That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
