RESOLUTION NO. RS2008-435
A resolution approving an extension and amendment by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and Catholic Charities of Tennessee, Inc., to provide care coordination for the Bridges to Care Program.
WHEREAS, Metropolitan Charter Section 10.104 provides that the Metropolitan Board of Health has the duty to contract for such services as will further the program and policies of the Board, subject to confirmation by Resolution of Council; and,
WHEREAS, the Metropolitan Board of Health and Catholic Charities of Tennessee, Inc. entered into a contract with the Bridges to Care Program to help improve access to medical, dental, mental health and substance abuse care for residents of Davidson County who have no or inadequate health insurance coverage, which was approved by RS2007-32; and,
WHEREAS, the Metropolitan Board of Health and Catholic Charities of Tennessee, Inc. wish to extend their contract with the Bridges to Care Program for an additional year, to begin July 1, 2008 and terminate on June 30, 2009 and to amend the contract to set compensation in an amount not to exceed $340,000; and,
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUTY:
Section 1. That the amendment to the contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and Catholic Charities of Tennessee, Inc., to provide care coordination for the Bridges to Care Program to help improve access to medical, dental, mental health and substance abuse care for residents of Davidson County who have no or inadequate health insurance coverage, a copy of which amendment is attached hereto and incorporated herein, is hereby approved and the Metropolitan Mayor is authorized to execute the same.
Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
