RESOLUTION NO. RS2008-414

A resolution approving an amendment to a contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and United Neighborhood Health Services to provide care to patients of the Downtown Clinic for the Homeless.

WHEREAS, The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, previously entered into a contract agreement with United Neighborhood Health Services to provide care to patients of the Downtown Clinic for the Homeless; and,

WHEREAS, the parties wish to amend that contract agreement to change the contract ending date from March 31, 2009 to October 31, 2008, and Paragraph 3 - Compensation, a copy of which Amendment I is attached hereto; and,

WHEREAS, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that Amendment I be accepted.

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That Amendment I to the contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, and United Neighborhood Health Services to provide care to patients of the Downtown Clinic for the Homeless, a copy of which Amendment I is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
