A resolution approving a contract agreement by and between The Metropolitan Government of Nashville and Davidson County by and through the Metropolitan Board of Health and Catholic Charities of Tennessee, Inc., to provide care coordination for the Bridges to Care Program.

WHEREAS, the Metropolitan Board of Health and Catholic Charities of Tennessee, Inc. wish to enter into a contract agreement for provision of services related to the Bridges to Care Program to help improve access to medical, dental, mental health and substance abuse care for residents of Davidson County who have no or inadequate health insurance coverage; and,

NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUTY:

Section 1. That the contract agreement by and between The Metropolitan Government of Nashville and Davidson County by and through the Metropolitan Board of Health and Catholic Charities of Tennessee, Inc. in an amount not to exceed $278,686.00, to provide a link for the uninsured to one of twenty-two consortium primary care clinics, a copy of which is attached hereto and incorporated herein, is hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
