A resolution approving an assignment and extension of an agreement by and between the Safety Net Consortium of Middle Tennessee, LLC and the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health.

WHEREAS, the Safety Net Consortium of Middle Tennessee, LLC, and the Metropolitan Government of Nashville and Davidson County, acting through the Metropolitan Board of Health, previously entered into a Memorandum of Agreement to integrate and coordinate the services of health care providers in middle Tennessee in order to more effectively provide health services to the uninsured; and

WHEREAS, that agreement was approved by Resolution No. RS2005-902; and

WHEREAS, the parties wish to assign that agreement; a copy of which assignment is attached hereto.

WHEREAS, the parties wish to extend that agreement; a copy of which extension is attached hereto.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the assignment by Safety Net Consortium of Middle Tennessee, LLC, of its rights, responsibilities and liabilities under the agreement to St. Thomas Health Services Fund, a copy of which is attached hereto and incorporated herein, is hereby approved.

Section 2. That the extension by the parties of the agreement for an additional contract term, a copy of which is attached hereto and incorporated herein, is hereby approved.
