A resolution approving a professional services contract, and an extension of such contract, by and between the Metropolitan Government of Nashville and Davidson County and Terry L. Edwards, Licensed Psychologist, for services to homeless persons.

Whereas, Terry L. Edwards has been selected to provide psychological testing and assessment services, substance abuse treatment services, and mental health counseling services to homeless clients of the Downtown Clinic; and

Whereas, the Board of Health executed a professional services contract with Ms. Edwards, as attached hereto, which contemplates the extension of the contract, as also attached hereto.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the contract and the contract extension by and between Terry L. Edwards and the Metropolitan Government of Nashville and Davidson County, acting by and through the Board of Health, to provide psychological testing and assessment services, substance abuse treatment services, and mental health counseling services to homeless clients of the Downtown Clinic, as attached hereto and incorporated herein, are hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
