A resolution approving a contract by and between Foundations Associates, Inc., and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide residential treatment services and half-way house placement to homeless persons with co-occurring disorders of mental illness and chemical dependency.

Whereas, Foundations Associates, Inc., in accordance with the contract attached hereto, wishes to provide residential treatment services and half-way house placement to homeless persons with co-occurring disorders of mental illness and chemical dependency; and

Whereas, this contract shall terminate on June 30, 2006 and may be extended for two additional years thereafter; and

Whereas, in exchange for the provision of residential treatment services and half-way house placement by Foundations Associates, Inc., the Metropolitan Government will provide payment of $80,000.00 for the first contract term.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the contract by and between Foundations Associates, Inc., and the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to provide residential treatment services or half-way house placement to homeless persons with co-occurring disorders of mental illness and chemical dependency, a copy of which is attached hereto and incorporated herein, is hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
