RESOLUTION NO. RS2005-902

A resolution approving an agreement by and between the Safety Net Consortium of Middle Tennessee, LLC and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to integrate and coordinate the services of health care providers in middle Tennessee in order to more effectively provide health services to the uninsured.

Whereas, the Safety Net Consortium of Middle Tennessee, LLC, wishes to more effectively provide health services to the uninsured by integrating and coordinating the services of the health care providers in middle Tennessee; and

Whereas, the Metropolitan Public Health Department currently owns and maintains an electronic registration, tracking, and referral system (the "Database") to coordinate health care services provided to the uninsured of Davidson County through the Bridges to Care program; and

Whereas, the Safety Net Consortium of Middle Tennessee, LLC wishes to cooperate in maintaining and entering information into the Database in order to provide more effective and extensive health care services to the uninsured of Davidson County; and

Whereas, the agreement attached to this resolution reflects that service, and institutes privacy and confidentiality safeguards on the transmission of protected health information; and

Whereas, this contract shall terminate on April 30, 2006 and may be extended for four (4) additional years thereafter; and

Whereas, there is no cost to the Metropolitan Government in exchange for these services.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the agreement by and between the Safety Net Consortium of Middle Tennessee, LLC and the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, to integrate and coordinate the services of health care providers in middle Tennessee in order to more effectively provide health services to the uninsured, a copy of which is attached hereto and incorporated herein, is hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
