RESOLUTION NO. RS2005-728


A resolution approving a contract by and between the Metropolitan Board of Health and the State of Tennessee, Department of Health, for the purchase of behavioral risk factor surveillance survey interviews.

WHEREAS, the Metropolitan Board of Health wishes to contract with the State of Tennessee, Department of Health, for the purchase of behavioral risk factor surveillance survey interviews of Davidson County residents; and

WHEREAS, this Contract initially expires on June 30, 2006, and may be extended for four additional years; and

WHEREAS, this Contract provides for payment to the State of Tennessee, Department of Health, at a rate of $21.50 per completed interview.

Now, therefore, be it resolved by the Council of The Metropolitan Government of Nashville and Davidson County:

Section 1. That the contract by and between the Metropolitan Board of Health and the State of Tennessee, Department of Health, for the purchase of behavioral risk factor surveillance survey interviews, a copy of which is attached hereto and incorporated herein, is hereby approved.

Section 2. That this resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
