RESOLUTION NO. RS2004-492

A resolution approving a second amendment to a grant from U.S. Department of Health and Human Services to the Metropolitan Government of Nashville and Davidson County, by and through the Board of Health, to improve and expand access to substance abuse, mental, dental, and other health services for uninsured residents of Davidson County.

WHEREAS, the Metropolitan Government of Nashville and Davidson County previously entered into a Grant Agreement with the United States Department of Health and Human Services to improve and expand access to substance abuse, mental, dental, and other health services for uninsured residents of Davidson County, which Grant Agreement was approved by RS2003-101; and

WHEREAS, the parties wish to first amend that Grant Agreement to increase the amount to a total of $571,489, which Amendment One has been submitted to Council for its approval; and

WHEREAS, the parties wish to further amend that Grant Agreement to increase the amount by $63,280, to a total amount of $634,769, a copy of which Amendment Two is attached hereto; and

WHEREAS, it is to benefit the citizens of The Metropolitan Government of Nashville and Davidson County that this Amendment be approved.

NOW, THEREFORE BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

Section 1. That Amendment Two to the grant by and between the U.S. Department of Health and Human Services, and the Metropolitan Government of Nashville and Davidson County, by and through the Board of Health, to improve and expand access to substance abuse, mental, dental, and other health services for uninsured residents of Davidson County, a copy of which Amendment is attached hereto and incorporated herein, is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That the amount of this grant be appropriated to the Health Department based on the revenues estimated to be received and any match to be applied.

Section 3. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
