RESOLUTION NO. RS2004-239

A resolution approving an amendment to a grant agreement by and between the Meharry Medical College and the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Department of Health, under which the latter is to conduct research regarding Asthma Disparities in Cohorts at Risk for Morbidity, extending the duration thereof and providing for an additional grant in the amount of $18,992.

Whereas, the Meharry Medical College and the Metropolitan Government of Nashville and Davidson County had an original agreement, a copy of which is attached hereto as Exhibit B, that the Metropolitan Department of Health would conduct research on Asthma Disparities in Cohorts at Risk for Morbidity and would receive a grant in the amount of Eighteen Thousand, Nine Hundred Ninety-Two Dollars ($18,992.); and

Whereas, the parties wish to extend the duration of the original grant agreement for an additional year, with additional grant monies to be distributed, and the original agreement must be amended to reflect that extension and addition.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the amendment to the grant agreement regarding Asthma Disparities in Cohorts at Risk for Morbidity, by and with Meharry Medical College, as attached hereto as Exhibit A and incorporated herein, to extend the duration of the same for an additional year, and to grant to the Metropolitan Government of Nashville and Davidson County, acting by and through the Department of Health, an amount not to exceed Eighteen Thousand, Nine Hundred Ninety-Two Dollars ($18,992.), is hereby approved, and the Metropolitan Mayor is authorized to execute the same.

Section 2. That the amount of this grant shall be provided to the Metropolitan Department of Health in the fiscal year 2004 / 2005, based on the revenues estimated to be received and any match to be applied.

Section 3. This resolution shall take effect from and after its passage, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
