A resolution approving a grant in the amount of $60,000.00 to the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, from the Department of Health of the State of Tennessee for administering telephone surveys to assess the knowledge and attitudes regarding influenza immunization and disease.

Whereas, the Health Department employees will conduct telephone surveys and will manage the data obtained; and,

Whereas, it is to the benefit of the citizens of the Metropolitan Government of Nashville and Davidson County that this contract be approved.

Now, therefore, be it resolved by the Council of the Metropolitan Government of Nashville and Davidson County:

Section 1. That the grant in the amount of $60,000.00 to the Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, from the Department of Health of the State of Tennessee for administering telephone surveys to assess the knowledge and attitudes regarding influenza immunization and disease for a term commencing on February 1, 2004 and extending through June 30, 2004, a copy of which grant contract is attached hereto and made part of this resolution be, and the same is, hereby approved.

Section 2. That this Resolution shall take effect from and after its adoption, the welfare of the Metropolitan Government of Nashville and Davidson County requiring it.
