RESOLUTION NO. RS2001-503

A resolution approving a Letter of Agreement by and between Volunteer State Health Plan, Inc. and The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health, for the Metropolitan Health Department to provide Disease Management Services to BlueCare members, to consist of Asthma, Diabetes, and Prenatal Education, with payment in accordance with the attached fee schedule.

Whereas, Volunteer State Health Plan, Inc. wishes to contract with the Metropolitan Board of Health for the Metropolitan Health Department to provide Disease Management Services to BlueCare members, to consist of Asthma, Diabetes, and Prenatal Education; and,

Whereas, if said Program is successful, the parties mutually agree to work in good faith toward a formal contract no later than sixty (60) days after the effective date of the Agreement; and,

Whereas, it is to the benefit of the citizens of The Metropolitan Government of Nashville and Davidson County that the Letter of Agreement be approved.

Now, therefore, be it resolved by the Council of The Metropolitan Government of Nashville and Davidson County:

Section 1. That the Letter of Agreement by and between the Volunteer State Health Plan, Inc. and The Metropolitan Government of Nashville and Davidson County, acting through the Metropolitan Board of Health, for the Metro Health Department to provide disease management services for Asthma, Diabetes, and Prenatal Education to all BlueCare members, and for a term as set forth in said Agreement, a copy of which is attached hereto and incorporated herein, be and the same is hereby approved and the Metropolitan Mayor is authorized to execute said agreement.

Section 2. That any amendments to this contract shall be approved by resolution by twenty-one affirmative vote of the Metropolitan Council.

Section 3. That this Resolution shall take effect from and after its final passage, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
