RESOLUTION NO. R99-1492

A resolution approving an amendment to a contract between The Metropolitan Government of Nashville and Davidson County acting by and through the Board of Health and Phoenix Healthcare of Tennessee, Inc., for administration of transportation services for TennCare enrollees which contract was approved by the Metropolitan Council through R98-1021.

Be it resolved by the Council of The Metropolitan Government of Nashville and Davidson County:

Section 1. That the contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health and Phoenix HealthCare of Tennessee, Inc., for administration of transportation services for all qualifying TennCare recipients in the Metropolitan CSA region is amended by deleting section C.1. Term in its entirety and substituting in lieu therefore:

C.1. This contract shall become effective as of January 1, 1998 and shall continue in effect until December 31, 1999.

which amendment to the contract is attached hereto and made part of this resolution be, and the same is, hereby approved.

Section 2. That the contract by and between The Metropolitan Government of Nashville and Davidson County, acting by and through the Metropolitan Board of Health and Phoenix HealthCare of Tennessee, Inc., is amended by changing the name of Phoenix HealthCare of Tennessee, Inc to Xantus Healthplan of Tennessee Inc.

Section 3. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
