RESOLUTION NO. R98-1205
A RESOLUTION AMENDING RESOLUTION NO. R93-990 TO APPROVE AMENDMENT NO. 4 TO A CONTRACT BY AND BETWEEN OMNI HEALTH CARE PLAN AND THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY BY AND THROUGH THE BOARD OF HEALTH.
BE IT RESOLVED BY THE COUNSEL OF THE METROPOLITAN GOVERNMENT AND DAVIDSON COUNTY:
SECTION 1. That Resolution No. R93-990 be amended by approval of Amendment No. 4 to the Contract approved by said resolution between the State of Tennessee, Department of Health, and The Metropolitan Government of Nashville and Davidson County, by and through the Metropolitan Board of Health and Omni Health Care Plan whereby the term of said contract is extended from January 1, 1998 through December 31, 1998, is amended and extended through those dates. All other terms and conditions remain the same, which Amendment No. 4 is attached hereto and made a part of this resolution, be and the same is, hereby approved.
SECTION 2. That this resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
