RESOLUTION NO. R98-1204

A RESOLUTION AMENDING RESOLUTION NO. R98-1174 TO APPROVE AMENDMENT NO. 5 TO GRANT Z-6-074146-6-00 BETWEEN THE STATE OF TENNESSEE, DEPARTMENT OF HEALTH AND THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY THROUGH ITS HEALTH DEPARTMENT (DOWNTOWN CLINIC).

BE IT RESOLVED BY THE COUNCIL OF THE METROPOLITAN GOVERNMENT OF NASHVILLE AND DAVIDSON COUNTY:

SECTION 1. That Resolution No. R98-1174 be amended by approval of Amendment No. 5 to Grant Z-6-074146-6-00, approved by said resolution, between the State of Tennessee, Department of Health and The Metropolitan Government of Nashville and Davidson County, by and through its Metro Health Department (Downtown Clinic), whereby Section E.8. for the period beginning July 1, 1997 is deleted with additions as reflected in the attached Amendment substituted in place.

Further, the following for the period beginning July 1, 1997 shall likewise be deleted:

WHEREAS, on or about the first day of January, 1998, it is the intent of the Department to change the manner in which the Department secures alcohol and drug abuse treatment services and initiate a fee for service methodology.

The other terms and provisions not amended hereby shall remain in full force and effect. Amendment No. 5 is attached hereto and made a part of this resolution, and the same is hereby approved.

SECTION 2. This resolution shall take effect from and after its adoption, the welfare of The Metropolitan Government of Nashville and Davidson County requiring it.
